I think Mayiladuthurai To Karaikal can be reached via kollumangudi , palayarai, thirunallar karaikal. But shivagama sundari answer may be correct because ; you can get more bus service thorugh peralam route.

Mayiladuthurai To Karaikal route goes via some great shiva temples. If you are shiva devote you can visit valur veratanam located on the way from mayiladuthurai to elanthankudi; then you can visist the great ThiruMeeyachur siva and Lalithambigai temple located near peralam(2.5km away from peralam); The lord dharpaneswar (thirunallar sani temple) located in Thirunallar, Karaikal ammaiyar temple near karaikal old bus stand.

There is no train track between Mayiladuthurai To Karaikal so no train server between these two towns. The nearest railway station is Nagore, nagapattinam, mayiladuthurai. But frequent bus service available to karikal throug akkur route, sankaran pandal route, nedungadu route etc. Best suggestion use the government bus service to reaach karikal and thirunallar
